Patient # 1 was an 83-year-old Caucasian male with a history of severe, O2 dependent, chronic obstructive pulmonary disease. He presented to the hospital with fever, dyspnea and productive cough. Chest radiograph on admission revealed evidence of diffuse emphysema and right-upper-lobe consolidation with collapse of several bullae. A chest CT confirmed bilateral bullous changes and air space consolidation in the right upper lobe. This patient was treated empirically for community-acquired pneumonia with resolution of his symptoms. Following treatment, the patient reported decreased dyspnea. Repeat chest radiography revealed persistent collapse of several right upper lobe bullae with scarring. Table 1 presents the results of pulmonary function testing and arterial blood gas sampling of this patient at baseline and after “inflammatory auto-bullectomy.”
